What Kind Of Documentation?
Diagnosing Professional - Your documentation must come from a qualified professional. This could be a medical doctor, a licensed therapist, an educational diagnostician, a psychologist or psychiatrist. Documentation needs to be from someone whose specialization allows them to make the determination or to treat the disability diagnosed, someone who has examined you and is not related to you.

Other Notes - If you are coming from a high school, your IEP or 504 Plan will not be accepted solely as documentation of your disability. If submitted with your documentation, these plans can provide helpful information regarding accommodations you have used in the past.
