What Do I need to View the Forms?
Forms are provided in PDF format which requires the free Adobe Acrobat Reader software (version 3.0 or higher) to view, fill, and print forms on-line with your web browser. Download the newest copy of Adobe Acrobat Reader here.

What Is a "Fillable" Form and How Do I Use One?
"Fillable" forms allow you to enter text directly onto the electronic copy of the form, via your keyboard and mouse. To use these forms...

  • Download and install the Adobe Acrobat Reader software.
  • On the Admission Form's page, click on the form to view. Form will open in your Acrobat software.
  • Use the mouse to navigate amoung the various pages of the form.
  • Choose to either print the document and hand write-in the necessary information, or "fill" the document electronically and then print.
    To fill the document...
  • Select the field to enter information by placing your mouse cursor over the corresponding text area and clicking.
  • Key-in desired information (use the "TAB" key to move to the next fillable field)
  • Print the filled form when finished and send to LBCC Admissions Office. (NOTE: It is necessary to click outside of the last entered field before you print your document, otherwise this field will not be printed).

Can I Save My Form?
Forms can be saved as PDF files to your machine for filling or printing at a later date, however, any text that you enter on-line will not be saved when the form is saved (this includes partially or completely filled forms). Be sure and print the form before you close it if you need access to this information again at a later date.

Why Won't a Form Field Print?
If you've "filled" in your form, you must make sure that you have clicked outside of the last field that you have typed in order to "set" the data. This then allows the everything you've typed to be printed.

Why am I having Problems Loading a Form?
You must have the Adobe Acrobat Reader (version 3.0 or higher) software installed to properly access the admissions forms. If you still have problems, file an error report by clicking here.
